Please bring any past medical records (lab results, imaging, list of current medications and supplements). You can also upload them to your Charm patient portal and share them with your provider to view before your first visit.
We require at least 48 hours' notice for new patients and 24 hours' notice for existing patients if you need to cancel or reschedule. This allows us to offer your appointment time to another patient in need.
